It could be allergies. My cat had a similar problem---loosing hair on her lower back..she was actually itching pretty bad from allergies and pulling her own hair out. My vet gave her a shot once a month that cleard it up.
It can be caused by anything from flea allergies to mange and of course being a stray it can be through poor diet but it most likely is fleas as it often causes patchy fur loss near the lower back as the cat can groom there more than anywhere as its easy to reach,
and the scratches to the ears and head could be where he has been scratching himself or been fighting other cats for food and females,
cats can have fur loss that's caused by hormones too so unless you see the state of the skin it wont be easy to tell, but if you do get close enough to see put gloves on before touching as you don't know what your dealing with.
They have to stare at your ugly face. Try preventing this problem by putting a brown paper bag over your head. Or you could just contact this helpline on 1800 40 40 10.
